The title of Outstanding SMALL Business of the Year is not easily won. In order to be recognized for this Honor, a business must have contributed to the Hueytown community through service to the community. Tell us how this nominee has made the City of Hueytown a better city in which to work, play and live. Each nomination should reflect information from various areas of the nominee’s participation, e.g., educational programs, Church /Civic programs, etc., or contribution to the community as a whole.

To be eligible for consideration, nominee must be a non-profit organization and a member in good standing of the Hueytown Area Chamber of Commerce, with a minimum of two complete years in operation. Chamber members may nominate a business, including their own, for consideration. Evaluation and selection of the Not-For-Profit Organization of the Year Award will be conducted by the Chamber Board of Directors. Board members with any conflicts will recuse themselves from the judging process.
Outstanding citizens are those who have made contributions that are “over and above” what is expected. A person of good character who exhibits a variety of interests, activities and areas of community support compiled over many years exemplifies the title of Outstanding Citizen of the Year.
Attach individual letters of support or recommendation to this nomination form. Each letter should be signed by the submitter.
All nominations will be reviewed by a selection committee. Decisions will be based on active participation in a variety of community roles. Nomination for Citizen of the Year ideally includes letters of support from any number of individuals representing churches, clubs, sport groups, civic organizations, educational groups and other activities demonstrating the community spirit of this nominee. Individuals may provide letters of support for more than one nominee however support letters must be submitted individually along with a separate Nomination Form.
While all teachers strive to improve the lives of their students, only a special few will leave a legacy of excellence. This award is presented to a Hueytown area educator who has encouraged “Excellence in Education”. Educators are not confined to a classroom. Other educators such as Librarian, School Nurse, Counselor, Coach or Principal may be considered.
